The story begins when a couple, Denise (Erin Daniels) and Jerry (Chris Hardwick), travel to the countryside to investigate a strange tale of local murders. They stop at a roadside museum called "Captain Spaulding's Museum of Monsters and Madmen", run by a clown-costumed Captain Spaulding (Sid Haig). Captain Spaulding is enthusiastic about telling them stories about a local murder trilogy, which includes the Firefly family horror drama.
Denise and Jerry decide to continue their exploration and search for the local legendary burial site of Dr. Satan. They stray off the beaten path and encounter a terrifying umbrella-carrying woman, Baby Firefly (Sheri Moon Zombie), who introduces them to her mother, the matriarch of the Firefly family, Mama Firefly (Karen Black). They soon find themselves trapped in the family's dark and twisted world, where sadism and murder are commonplace.
The film continues in a suffocatingly terrifying and brutal setting as Denise, Jerry and their friends attempt to escape the clutches of the Firefly family. They encounter various notorious family members, including the extremely disturbed Otis Driftwood (Bill Moseley) and the psychopathic Rufus Firefly (Robert Mukes).
